Terms of Service
The terms that apply to this website and to membership with Momento Medical.
Last updated October 25, 2026. These terms apply to this website and to membership with Momento Medical LLC. By using the site or enrolling as a member, you agree to them. If you sign a separate membership agreement at enrollment, that agreement controls if the two ever conflict.
Who we are
Momento Medical is a direct primary care practice in Greenville, South Carolina, led by a board-certified nurse practitioner. Members pay a flat monthly fee for the primary care services described on our Services page.
Membership is not insurance
Your membership is an agreement between you and your provider for primary care. It is not health insurance, it is not a health plan, and it does not satisfy any state or federal requirement to carry insurance. We do not bill insurance for services covered by your membership. You should keep separate coverage for specialist visits, hospital care, imaging, emergency services, and anything outside the scope of primary care.
Who can enroll
- You must be at least 18 years old to enroll yourself. A parent or legal guardian may enroll a minor.
- You agree that the information you give us at enrollment and afterward is accurate and that you will keep it current.
- We may decline or discontinue membership when we cannot safely meet a person's medical needs within primary care.
Fees and billing
- The monthly membership fee is billed in advance on a recurring basis. Current prices are listed on the Membership page.
- A one-time enrollment fee applies to Individual and Couple plans and covers setting up your record and your extended first visit.
- Listed prices reflect ACH bank payment. Other payment methods are accepted at the rate shown on the Membership page.
- We may change prices with at least 30 days' notice. Continuing your membership after the change takes effect means you accept the new price.
- If a payment fails, we will let you know. Membership may be paused until the balance is paid and may be ended if it remains unpaid.
What membership includes
Membership covers the primary care services described on our Services page, including office and telehealth visits with no per-visit fee, same or next day appointments when available, direct text and phone access to your provider for non-urgent needs, and chronic condition management. Labs and medications are offered at near-cost or discounted pricing and are billed separately unless stated otherwise. We may update the included services from time to time and will post changes on the website.
What membership does not include
- Emergency room visits, ambulance services, and hospital care.
- Specialist visits and care outside primary care.
- Imaging such as X-ray, MRI, and CT, which we refer out.
- Prescription medications, unless stated otherwise.
- Surgery and procedures we do not perform.
You are responsible for the cost of any service you receive from another provider, even if we referred you.
Emergencies
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use text, email, or voicemail for emergencies. These channels are for non-urgent needs and are not monitored around the clock.
Communication and response times
Direct access means you can reach your provider by text, phone, or secure message without going through a call center. We make every effort to respond promptly during business hours, but we cannot guarantee an immediate reply. Messages received outside business hours are usually answered the next business day.
Text messages, email, and phone calls
- By giving us your mobile number, you agree that we may contact you by text message, including appointment reminders, lab results, care instructions, and account notices. Message frequency varies. Message and data rates may apply.
- You may stop text messages at any time by replying STOP, and get help by replying HELP. Agreeing to texts is not a condition of membership or care.
- Standard text messages and email are not fully secure. By using them, you accept that risk. If you prefer, ask us to use a different method.
Telehealth
Telehealth visits are included when your provider decides they are appropriate. A virtual visit has limits compared with an in-person exam, technical problems can interrupt a visit, and your provider may ask you to come in. Telehealth is available to members who are physically located in South Carolina at the time of the visit.
Your responsibilities as a member
- Give accurate and complete health information and tell us about changes.
- Follow the care plan you agree to with your provider, or tell us when you cannot.
- Keep your appointments or let us know ahead of time when you need to reschedule.
- Treat our staff with respect.
Cancelling and ending membership
- Membership is month to month. You may cancel at any time, and your membership ends at the close of the current billing period. Fees already paid are not refunded.
- You are welcome to re-enroll later. A new enrollment fee may apply.
- We may end a membership for non-payment, repeated missed appointments, abusive or threatening behavior, or when your medical needs are beyond what primary care can safely manage. We will give you written notice and help you transfer your records.
No guarantee of results
We provide care based on current clinical standards and professional judgment. We cannot guarantee a specific outcome. Information on this website is general and is not a substitute for a visit with your provider.
