A one-time $99 enrollment fee applies to Individual and Couple plans. All plans are HSA/FSA-eligible. Prices reflect ACH payment. Other payment methods accepted at 4% higher.

Membership is not insurance
- An agreement between you and your provider for the primary care services on this site.
- Not health insurance, and it does not satisfy any state or federal requirement to carry insurance.
- Month to month, with no claims, copays, or surprise bills.
- Many members pair it with a high-deductible plan for everything else.
Keep coverage for the rest
These are not included in the monthly fee. We coordinate referrals when you need them.
