Privacy Policy
How we handle your information on this website and as a patient of Momento Medical.
Last updated October 25, 2026. This page has two parts. The first explains how this website handles the information you share with us. The second is our Notice of Privacy Practices, which explains how we protect your health information once you become a patient.
Website Privacy
What this website collects
- Contact form submissions: your name, email address, phone number, and the message you write. Our forms are processed by Formspree, a form service that receives the submission and forwards it to our office email.
- Anything you send us directly by phone, email, or text when you reach out.
- Your language preference, which is stored in your own browser so the site can remember whether you prefer English or Spanish. This is not a tracking cookie and is never sent to us.
- Basic technical information that any web host records when a page is served, such as the date, time, and the type of browser used.
What this website does not do
- We do not run advertising trackers, analytics pixels, or social media tracking on this site.
- We do not sell, rent, or trade personal information.
- We do not share mobile phone numbers or text messaging consent with any third party for marketing purposes.
How we use what you share
We use contact information only to answer your questions, schedule visits, and complete enrollment if you decide to join. If you are not a patient, we keep your message only as long as needed to respond and follow up.
Service providers
A small number of companies help us run this website and our practice, such as our form service, our web host, and our electronic health record system. They may handle your information only to provide their service to us, and they are required to protect it.
Links to other sites
This site may link to outside websites, such as a scheduling tool or a payment page. Those sites have their own privacy practices, and we encourage you to read them.
Children
This website is intended for adults. A parent or legal guardian must enroll any member under 18 and handles that member's information.
Your choices
You may ask us at any time to delete a message you sent through the website, to stop contacting you, or to correct your contact details. Use the contact information at the end of this page.
Notice of Privacy Practices
This notice describes how medical information about you may be used and shared, and how you can get access to it. Please review it carefully. It applies to all records of your care that Momento Medical creates or keeps.
Our responsibilities
- We are required by law to keep your protected health information private and secure.
- We must give you this notice of our legal duties and privacy practices, and follow the version that is currently in effect.
- We will let you know promptly if a breach occurs that may have compromised your information.
- We will not use or share your information other than as described here unless you tell us in writing that we may. You can change your mind at any time by telling us in writing.
How we typically use and share your health information
- To treat you. For example, to order labs, prescribe medication, coordinate a referral, or share records with another provider involved in your care.
- To run our practice and bill for membership. Because we are a direct primary care practice, we bill you directly and do not submit claims to insurance for services covered by your membership. We may use your information for quality improvement, staff training, and administration.
- To stay in touch with you. We may call, text, or email you about appointments, lab results, refills, and care instructions, using the contact methods you have agreed to.
- With people involved in your care. With your permission, or when you are unable to object and we believe it is in your best interest, we may share relevant information with a family member, caregiver, or someone you have designated.
Other situations where the law allows or requires sharing
- Public health and safety, such as reporting communicable diseases, adverse drug reactions, or suspected abuse or neglect.
- Health oversight activities such as audits, inspections, and licensing reviews.
- Court orders, subpoenas, and other lawful requests in legal proceedings.
- Law enforcement requests that meet legal requirements.
- Coroners, medical examiners, and funeral directors.
- Organ and tissue donation organizations.
- Research, only under strict conditions and with proper safeguards or your authorization.
- To prevent a serious and imminent threat to someone's health or safety.
- Specialized government functions such as military, veterans, and national security activities.
- Workers' compensation and similar programs.
Uses that always require your written permission
- Marketing communications that are not about your own care.
- Any sale of your health information.
- Sharing psychotherapy notes, where they exist.
- Any other use or disclosure not described in this notice.
Your rights
- See and get a copy of your record. Ask us in writing and we will provide an electronic or paper copy, usually within 30 days. We may charge a reasonable, cost-based fee.
- Ask us to correct your record if you believe something is wrong or missing. We may decline, and we will tell you why in writing within 60 days.
- Ask us to contact you a specific way, for example only by email or only at a certain number. We will honor reasonable requests.
- Ask us to limit what we use or share. We are not required to agree, but if you pay for a service in full out of pocket, you may require us not to share information about that service with a health plan for payment or operations purposes, and we will honor that request.
- Get a list of the times we have shared your information for purposes other than treatment, payment, and operations, for up to six years before your request.
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Choose someone to act for you, such as a medical power of attorney or legal guardian. We will confirm their authority before acting on their requests.
- File a complaint if you feel your rights were violated. You may complain to us using the contact information below, or to the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
Text messaging
If you agree to receive text messages from us, your phone number and your consent are used only to communicate with you about your care and your membership. They are not shared with third parties or affiliates for marketing. You may stop texts at any time by replying STOP.
Changes to this notice
We may update this notice, and any change will apply to information we already hold as well as information we receive in the future. The current version is always posted on this page and available on paper at our office.
